benefits your company offers. This is important to use in order to
document the benefits were offered and the employee turned them down.
You should keep a copy of this form in the employee file.

for group medical and/or dental.

Employee apps are used although the Employee dental app is only needed
if dental is added later or as a stand alone coverage. The normal
employer medical app allows enrollment in dental.
